
How Cavities Actually Form, and Why They Do Not Hurt at First
Almost everyone has had the experience of being told they have a cavity in a tooth that feels completely fine.
It is a strange moment. You trust your own body, and your body is reporting nothing. So the finding feels like a matter of opinion, and the fix feels optional.
It is not a matter of opinion, and the reason it is painless is the most useful thing to understand about teeth.
Sugar does not dissolve your teeth. Acid does.
The bacteria living on your teeth eat the same carbohydrates you do. Feeding produces acid as a by-product, and that acid pulls minerals out of the enamel surface. This is demineralisation, and it starts within a couple of minutes of eating.
Your saliva reverses it. Saliva neutralises acid and carries calcium and phosphate, which it returns to the enamel surface. That is remineralisation, and it is a genuine repair process running in your mouth every day.
A cavity is simply the point where demineralisation has outpaced remineralisation for long enough. Not a single event. A balance that tipped and stayed tipped.
This is why the advice you have been given about sugar is usually the wrong shape.
Frequency beats quantity, and it is not close
Here is the part that changes behaviour once people understand it.
Every time acid arrives, the mouth stays acidic for roughly twenty to forty minutes before saliva brings it back to neutral. Saliva needs that uninterrupted window to do the repair work.
So consider two people who consume exactly the same amount of sugar in a day.
The first drinks a can of soda with lunch, in ten minutes. One acid attack. Hours of repair time afterwards.
The second sips the same can slowly from nine until noon. The mouth never returns to neutral. Repair never gets its window.
Same sugar. Completely different outcome. The grazer will get cavities and the gulper mostly will not, and the grazer will feel unfairly treated, because by the usual measure they did nothing worse.
This is also why a habit as small as a sweetened coffee kept on a desk all morning does more damage than dessert.
Why it is silent until it is expensive
Now the part that explains the painless cavity.
Enamel is the outer layer, and enamel has no nerve supply at all. Decay can work through it for a year or two and there is nothing there to report it.
Underneath is dentine, softer and threaded with microscopic tubes running toward the centre. Decay moves faster here, and this is where sensitivity to cold or sweetness can start.
At the centre is the pulp, the living tissue holding the nerve and blood supply. Real toothache begins when decay reaches it, and the pain has a specific cause. The pulp becomes inflamed and swells, but it sits inside a rigid chamber with nowhere to expand and no drainage. Pressure builds against the nerve.
The part of a tooth that can feel pain is the last part decay reaches. So "nothing hurts" tells you where the problem has got to, not whether you have one.
The treatment ladder follows that sequence exactly. Early enamel decay can sometimes be remineralised with no drilling. Through the enamel means a filling. Into the pulp means a root canal and a crown, or an extraction. The gap between the first and the last is often eighteen months and one x-ray.
Why we take x-rays for something you cannot feel
A large share of decay begins on the surfaces between teeth, where they touch each other. Those surfaces cannot be seen in the mouth. Not hard to see. Impossible, for you and for me.
That is the whole argument for the images. They are how a problem gets found while it is still a filling.
What actually reduces your risk
Cut the number of sugar exposures, not just the amount. Sweet things with meals rather than between them. Water in between.
Fluoride works by changing the mineral itself. When enamel remineralises with fluoride present, it forms fluorapatite, which dissolves at a lower pH than the original mineral. The rebuilt surface is more acid-resistant than what was there before. This is why brushing at night matters most: saliva flow drops during sleep, so the fluoride is doing its work at the moment your natural defence is weakest.
Do not rinse after brushing. Spit, and leave it. Rinsing washes away the fluoride you just applied.
Clean between your teeth, because that is where the decay you cannot see is starting.
What to do
If it has been more than a year since your last x-rays, that is the gap worth closing. Decay found between teeth at the enamel stage is a small filling. The same decay found two years later is a very different appointment.
